Arizona State income tax forms 301 and 323 are used for claiming this tax credit. You can retrieve Arizona tax form 301 and 323 for 2011 from the Department of Revenue’s website: www.azdor.gov/Forms/Credits.aspx or click on the below links. Please note that the tax credit forms for 2011 are not available until late in the calendar year. The links for the forms are as follows:

Form 323 is only needed, if this is the only state tax credit you are claiming this year. If you are claiming more than one state tax credit, then Form 301 is additionally needed.
