Arizona Tuition Tax CreditArizona Tuition Organization’s 2011-12 Grant-Scholarship Application is consistent with Arizona’s Statutes regarding applicant requirements.  Financial information is required from each applicant’s household.  The financial reporting is now based on the Federal Reduced & Free Lunch Program guidelines. A worksheet detailing income from all members of an applicant’s household is the industry standard required format. Federal Tax Returns are not required.

The application consists of two pages. The first page must be completed for each child applying for a tuition award through AZTO. The second page is a Financial Information Form. You only need to fill out one financial form per family.

Grant/Scholarship Application (Page 1) consists of basic student contact and school information. Please include a current email address, as AZTO prefers to communicate in this manner. If your child attended another school in the prior school year, list the former school name. The narrative section includes student merit, benefits of private education, specific needs, and total family tuition expense. If you need additional space to write the narrative, please attach a separate piece of paper.

Financial Information Form (Page 2) requires families to list all members living in your household, all children and non-relatives living in your home. You will then list the income earned by each member based upon the prior month. You will need to multiply the individual’s income: Weekly by 52, Every 2 Weeks by 26, Twice a Month by 24, and Monthly by 12. Once this is completed for all members of the household, add the total annual income for each member of the household and list this as the Total Annual Household Income.
